Ending. The unique ending for Lich is throwing your Phylactery into the Worldwound to close it. The requirement for the ending is to have hit level 8 in Lich (corresponding to Mythic Level 10). You can do this ending even if Zacharius destroyed your phylactery as the only check is your Lich level. Fighting Zacharius.

All right, I found the answer to my question. For anyone having the same issue: The handkerchief is being removed automatically after the boss fight in Leper's Smile. However, it is not needed. All you need is the perfume, for which you have to pass the perception check.
